Our field has achieved remarkable progress in reducing disparities in health and healthcare, yet there’s still more to be done. This forum provides a valuable opportunity to exchange successes and challenges, learn from one another, and reaffirm our commitment to the crucial task of enhancing health and well-being for all members of our communities.


Connect and learn from health equity leaders from federal and local agencies, healthcare organizations, community-based organizations, academia, and others to actively promote and maintain health equity through action. Forum participants will be able to receive updates from leadership on recent initiatives and programs, delve into the latest health equity research, share promising practices and innovative solutions, and work together on community engagement strategies.



  • Examine the current social determinants influencing health challenges in our communities.

  • Identify organizational strategies and best practices aimed at addressing and bridging health disparities among patients and care providers.

  • Compare innovative models that focus on creating and safeguarding health equity.

  • Recognize comprehensive, efficient, and accessible services for all, wherever and whenever required.

  • Assess the unequal distribution of power and resources while improving daily living conditions.

  • Measure and monitor health outcomes and service delivery, while collaborating with other sectors to oversee patient’s living conditions.
