This session will highlight how state governments can implement statewide social determinants of health (SDOH) data collection, and make data actionable for community, healthcare, and public health stakeholders through SDOH data standards implementation. A panel discussion will include representatives from a multi-stakeholder initiative that develops consensus-driven data standards to support the collection, use and exchange of data to address SDOH at a Pennsylvania Health Information Organization (HIO). Panelists will provide insights on key partnerships, stakeholders, workflows, technological capabilities and community engagement efforts needed to scale standardized data collection. Presenters will also detail how the data is organized and used to support individual health and social care needs, and broader public health efforts. The panel will be facilitated by a representative of a technology company that partners with health systems, nonprofits, states and other stakeholders to connect all people in need to programs that serve them.
