Discover how advanced analytics is revolutionizing the identification and management of high-risk pregnancies. This presentation shares a case study from a Medicaid ACO that uses an innovative data-driven approach to improve maternal health outcomes. By integrating clinical best practices and social determinants of health (SDOH) into a risk stratification model, the program identifies and supports at-risk mothers from pregnancy through 12 months post-partum. The first year of implementation showed promising results, including a significant increase in prenatal visits and high enrollment rates.
