While providers are still early in the migration to value-based care, millions of Americans are already receiving care under fully accountable value-based care contracts. Achieving the shift on this scale has brought important learnings on consumer, patient, and health plan member engagement and the role of technology in enabling a more consumer-centered approach to care. In particular, two key pillars in technology strategy for value-based care have emerged in interoperability and home-based care. This session will detail the pivotal considerations for these terrains in driving change at scale across care delivery organizations to improve how healthcare is accessed, delivered, experienced, and made more affordable. For example, how can providers reduce complexity and friction so we can achieve a future where consumers avoid the anxiety, life disruption, and cost of an unnecessary emergency department visit by being empowered to better manage their own care.
