Orlando Health Network (OHN), a clinically integrated network serving over 300,000 patients and comprising more than 5,500 physicians, aimed to improve patient care and outcomes through a focus on care gap closure rates and increase annual wellness visits. This session will showcase OHN's data-driven approach, enabling physicians with real-time insights into open care gaps at the point of care. OHN's use of technology to assist physicians to address open care gaps during the active encounter and identify uncaptured comorbidities missed coding opportunities, OHN significantly improved patient care and outcomes' implemented a targeted outreach campaign via SMS and email, effectively engaging high-risk patients and boosting annual wellness visit reason achieved a 47 percent closure rate for care gaps for the providers that used the point of care technology surpassing non-adopters by nearly 8 percent. Additionally, OHN achieved a 44.2 percent annual wellness visit rate and a 28.9 percent completion rate for breast cancer screening visits, demonstrating substantial improvements in preventive care and patient engagement compared to prior manual outreach.
