In 2025, the Center for Program Integrity within the Centers for Medicare and Medicaid Services implemented the Fraud Defense Operations Center (FDOC). The purpose of this center was to address fraud risk in near real time by bringing together analysts, investigators, law enforcement, and compliance/regulatory experts. In total, the FDOC triaged over 340 suspect providers and took swift action to prevent payment of over $1.4 billion while fraud investigations continued. The FDOC serves as a model to prevent fraud losses through targeted analytics and data-driven decision making.
