

Modernizing Chronic Disease Population Health Analytics: On the Right TRAX

The Transformational Repository & Analytics eXchange (TRAX) is a collaboratively governed, public–private data-sharing initiative advancing chronic disease surveillance and population health in Washington State. Co-developed by the Washington State Department of Health and the Washington Healthcare Forum, TRAX enables secure, standards-based exchange of patient-level data—beginning with hypertension and diabetes—across providers, payers, and public health.
This session traces the TRAX journey from governance design to FHIR-enabled implementation, highlighting practical lessons in shared data ownership, policy alignment, and building trust among participating organizations. Attendees will learn how TRAX has progressed from piloting exchange with MultiCare via a TEFCA-qualified network to engaging new health systems, reducing administrative burden, and aligning with CMS-driven interoperability pathways.
The discussion will also explore how TRAX positions itself within emerging Health Data Utility (HDU) models—serving as a trusted intermediary for multisector data exchange—and how it aligns with CMS network initiatives that drive national consistency, scalability, and sustainability. By situating TRAX within these broader federal frameworks, Washington is shaping a replicable model for modernized and future-ready public health data infrastructure.
