

Scaling Care Continuity: Collaboration with Care Partners in Taiwan

This session presents National Cheng Kung University Hospital's (NCKUH) pioneering approach to care continuity through structured collaboration across Taiwan's healthcare ecosystem. As the first institution in Taiwan pursuing HIMSS Continuity of Care Maturity Model (CCMM) certification alongside its care partners, NCKUH demonstrates coordinated partnerships that enhance patient outcomes and system efficiency. Led by Superintendent Dr. Lee, this 1,342-bed medical center has developed frameworks for information sharing, care coordination, and referral management that strengthen care transitions. Attendees will explore practical strategies including technology enablement through HIMSS INFRAM advancement, standardized protocols, shared metrics, and governance structures supporting care delivery beyond hospital walls. Presenters will share insights on overcoming interoperability challenges, aligning stakeholder incentives, and measuring impact on patient outcomes in complex, multi-provider environments.
