Longevity Health Plan (LHP) implemented a data-driven care model to improve outcomes for high-risk populations while controlling costs. By deploying targeted protocols for acute conditions like urinary tract infections, LHP successfully reduced hospital readmissions. Focused interventions for dialysis patients and nursing home residents helped lower overall costs and prevent avoidable emergency visits. Personalized care journeys were developed for members with complex needs such as end-stage renal disease, diabetes and Alzheimer’s—improving care coordination and engagement. Additionally, integrating myelodysplastic syndrome assessments in nursing homes enabled real-time monitoring, reducing unnecessary hospital transfers and improving quality performance.
