This session showcases how one healthcare system improved health and medication literacy through a patient-centered education tool integrated into its electronic healthcare record. Attendees will learn how personalized, multilingual and standardized medication calendars using plain language, pictograms and clinical indications can help improve patient comprehension of their medication regimens and discharge readiness.
