

Implementation Strategies for Patient-Reported Outcome Measures: Three Case Studies

Patient-reported outcome measures (PROMs) capture the patient’s voice on symptoms, function and well-being, providing critical data often missing from routine clinical documentation. This session examines three distinct PROM implementation approaches: a system-wide academic medical center program, an oncology-focused community healthcare system initiative, and a digital healthcare platform serving outpatient rehabilitation. Each case study demonstrates different strategies for addressing common implementation barriers—clinician resistance, workflow disruption and technical integration challenges—while achieving measurable improvements in care coordination, symptom management and patient engagement. Presenters will share specific methodologies, including custom platform development, electronic healthcare record integration approaches, automated workflows, and patient engagement strategies that resulted in improved clinical outcomes and reduced emergency department visits. Attendees will learn key criteria for PROM implementation and practical strategies from multiple organizational models, enabling them to select and adapt approaches most suitable for their own organizational context and care delivery environment.
