Orlando Health Network established a care activation model that unites provider engagement, patient outreach and data-driven prioritization to close care gaps at scale. By embedding point-of-care prompts into physician workflows and automating outreach across varying populations, the network enabled rapid, measurable improvements without expanding staff. Providers using the system achieved a 28.2-percent relative improvement in quality gap closure. Outreach campaigns engaged more than 10,700 patients with 86.1-percent response rates and 34-percent gap closure, generating $842,000 in attributed visit revenue and $65,000 in workflow savings. A 29-percent year-over-year rise in documentation quality was also observed, with more than 40-percent closure rates sustained across chronic measures. This session shares how Orlando Health built a multichannel infrastructure for activating care, combining physician-facing tools, consumer engagement and real-time analytics to drive enterprise-wide performance.
