High readmissions and poor outcomes for Skilled Nursing (SNF) patients transferred to the ED are common. Effective information exchange and coordination between SNFs and ED staff is a continuing challenge. If the history packet is sent with the patient, it rarely provides quick and easy access to meaningful data for front line staff to render rapid decisions or treatment. However, innovative health systems like Community Health Network (CHN) are advancing care collaboration between their ED providers and post-acute network partners through the exchange of integrated real-time data on SNF transfer patients. ​
